I am writing to inform you of a difficult decision I had to make regarding participating insurance for Theresa Setter, NP Family Health PLLC, dba Revitalize Dermatology & Aesthetics.
As of July 1, 2024, we will no longer be participating with any insurance companies directly. This does not change our range of services. At your upcoming visit, we will require that payment be made at the time of service. Our fee schedule is listed below for your reference, and I have tried to keep the fees as reasonable and affordable as possible. After your visit, you will make payment, be provided a receipt as well as an encounter form for you to submit to your insurance for reimbursement. Unfortunately, we cannot guarantee reimbursement of fees paid because all plans differ based on co-insurance, deductible and plan.
We do offer payment plans through Care Credit at 0% for 6 months. Application is required for this and approval is based on your credit score.
In 2015, New York State passed the Nursing Modernization Act which would allow Nurse Practitioners, like me, to practice independently without a collaborating MD, provided we had 10 years or more experience in our specialty. I was proud to be one of the first in Syracuse to take this opportunity. Although this was a really important step for midlevel practitioners, limitations were placed that would soon cripple Nurse Practitioner-owned practices.
For example, our practices are only reimbursed at 80% of what MD practices are reimbursed. In addition, the reimbursement rates have decreased by 12% since 2016, while supply costs seem to be increasing steadily since the pandemic. Further, our billing costs have increased, and salaries of employees have had to increase due to cost-of-living increases.
As you can imagine, this has made it very difficult for small practices like mine to survive. I am forced to make a change for my employees and for the practice as a whole.
I know that this is a significant change. Unfortunately, it is a necessary one for my practice. Thank you to those of you who have been loyal to me through many changes and I hope that I will be able to continue to provide you with thorough Dermatologic care.
Should this not be a reasonable option for you, I would be happy to refer you to a new provider in our area and will send your records to the office of your choice.
Attention MEDICARE patients: Medicare DOES NOT allow for reimbursement for non-participating providers. You will need to sign a waiver at your upcoming appointment acknowledging this. This means that, for example, if you pay $100 for your annual skin cancer screening, you will not be able to submit to Medicare for reimbursement for any portion of this.

** Please note that if a specimen is sent to the lab for pathology, a copy of insurance will be sent to the lab as they bill separately. Most insurances are accepted by CBL; however, you may check with your insurance company or CBL directly to question your responsibility for payment regarding specimens sent.
What Does Emsella Treat?
The laser beams from the diVa laser target the lax vaginal walls, resurfacing them. As healthy, young, and elastic tissues replace the old and loose tissues, your vagina feels tighter and softer.
